Advising Forms


Metropolitan State has listed a number of commonly used academic advising forms below, allowing you to download and mail required forms to the appropriate advising office. If you are not sure which form(s) to use, or need other help with advising forms and procedures, contact a Metropolitan State University advisor.


Name Description
Major Declaration Form Business Online Students must declare their major after they have completed the online orientation.
Transfer Credit Evaluation Appeal Students should review the general education and liberal studies worksheet to make certain the transfer credits and equivalent courses are correct. If you disagree with the information on your GELS worksheet, you can appeal the transfer decision by completing this form and submitting it to the Metropolitan State Admissions Office. The review of your appeal may take several weeks.
Semester Equivalent Foundation and Business Courses A chart listing Twin Cities Community Colleges and the courses that are equivalent to Metropolitan State's Foundation and Core courses.
Waiver Examination
MIS 100

Econ 201, Econ 202
Acct 210, Acct 320
Waiver examinations are available for MIS 100, Econ 201, Econ 202, Acct 210, or Acct 320. If you have previous experience or previous course work that didn't transfer for these courses, you may take a free waiver examination to test out of the course. You can take the waiver examination for a course only once. You will not receive credits for passing the exam, but you will have fulfilled the course requirement and will not have to take the course.
Prior Competence Evaluation Proposal If you have prior experience or course work in an area, and believe it is equivalent to course requirements, you may apply for a prior competence evaluation. This may give you the opportunity to fulfill course requirements without having to take the course. This is the form you must have approved in order to receive credit for prior competence.

College of Management Prior Procedures

If you apply for a prior competence evaluation, the College of Management has specific procedures you must follow to apply for the Prior Competence Evaluation. This form is the procedural information.
Major Checklist This is a list of the required courses for the major. Advisors use this checklist to mark which course you have completed at previous institutions that fulfill the BS requirements, and which courses you must complete through Metropolitan State or a partner institution. The checklist can also be used to track your progress by filling in the semester and year you completed the course and the grade you received.
Repeated Course Form If a student receives a failing grade in a course at Metropolitan State, they can take that course again.
If a passing grade is received when the student repeats the course, the student or an advisor can fill out the repeated course form and send it to the records office. Both grades are recorded on the transcript, however, only the second passing grade is calculated into the GPA.
Outstanding Student Award The College of Management (COM) selects one graduating student each semester to be awarded outstanding student at Commencement. A GPA of 3.5 or higher is one of the criteria. The form lists the other criteria. A student can complete this form in their last semester and submit it to Kate Southwick, advisor for COM.
